In accordance with the order of the Ministry of Health of Turkmenistan No. 358 dated July 25, 2023, a certificate form was approved, which is issued if the mother of the child cannot provide a passport or other identity document.
Help contains the following information:
- Surname, name and patronymic of the mother of the child;
- Date of birth, nationality of the mother of the child;
- Place of birth of the mother of the child;
- Place of work of the mother of the child;
- Place of residence of the mother of the child;
- Name and signature of the father of the child;
- Gender and date of birth of the child;
- Data of the attending physician;
- Data of three doctors who were present at the birth of the child;
- Information of the head of the department;
- Data and signature of the head of the medical institution;
- Seal.
