In accordance with the order of the Ministry of Health of Turkmenistan No. 358 dated July 25, 2023, a certificate form was approved, which is issued if the mother of the child cannot provide a passport or other identity document.

Help contains the following information:

• Surname, name and patronymic of the mother of the child;
• Date of birth, nationality of the mother of the child;
• Place of birth of the mother of the child;
• Place of work of the mother of the child;
• Place of residence of the mother of the child;
• Name and signature of the father of the child;
• Gender and date of birth of the child;
• Data of the attending physician;
• Data of three doctors who were present at the birth of the child;
• Information of the head of the department;
• Data and signature of the head of the medical institution;
• Seal.
